Carillon Sports and Family Medicine strives to provide our patients with high quality, affordable medical care. Our office performs a courtesy benefits verification prior to each appointment and communicates this information via text or telephone. We accept a variety of payment options to meet your needs, including cash, Visa, MasterCard, American Express and Discover credit cards. If you have any questions, please ask us! We will thoroughly explain your choices and work hard to accommodate your needs.

INSURANCE POLICIES

Our office staff will be glad to assist you with your insurance coverage. When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer's plan, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment.

We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 45 days. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form. If you do not have insurance or we are not contracted with your insurance it's no problem! We offer affordable pricing to anyone seeking an office visit.
