What to expect

Please call our office to schedule your new patient appointment.  When you call, we will obtain demographic information as well as insurance information and enter it into our electronic medical records system.

We will ask that you print the new patient forms from this site and complete them.  If possible, please email or fax the forms to our office prior to your appointment.  Please arrive 15 minutes prior to your scheduled appointment time with your completed paperwork in order to allow us to finalize the patient registration process.    We will scan a copy of your picture identification and insurance card into our electronic medical records system.

Your copayment, coinsurance and/or deductible will be collected on the day of your visit.  Our computer system is linked with the insurance carrier fee schedules, so we are able to determine patient responsibility on the day of your visit.   Our office accepts cash, check, American Express, Discover, MasterCard or Visa.


Electronic Medical Records information

Our office is pleased to have been on the forefront of the implementation of electronic medical records system.  Our system was implemented in October 2005 and remains one of the most comprehensive and technologically advanced systems on the market today.

You may be wondering what benefits electronic medical records will offer you as a patient of our practice.  Here are a few examples:

  • Continuity of care – Your chart will always be up-to date, legible and accessible.  Your chart will always be at our fingertips and will experience no delays attributed to having to locate your chart.
  • Privacy – Your personal health information is accessible only to authorized individuals within our office.  All computers are password protected and the system is very secure.  No information is accessible via the Internet.
  • Timesaving – Your prescriptions will be sent electronically to your pharmacy, when available.  This will reduce your wait time at the pharmacy.
  • Increased efficiency – refills and referrals are more easily processed when all clinical information is easily accessible.
  • Safety – Many studies have proven that the use of electronic medical records improves safety due to features such as automatic allergy and drug interaction alerts.  Additionally, better organization and legibility of the chart in general lend to easier recognition of chronic medications and medical problems.
  • Improved outcomes – Built in prompts such as up-to-date disease management plans, follow up labs or imaging studies and preventive health service reminders assist with managing patient care.
